In order to provide care and services, we collect and hold information about our residents.
Under the Privacy Act 2020 and in accordance with our Privacy Policy, you can ask to receive a copy of the information held about you.
To ensure we maintain privacy and confidentiality, resident information will only be provided on receipt of a completed request form and associated documentation.
Please download the relevant application form and submit your completed form with the necessary documentation either:
- In-person: Care Home Manager, General Manager or Care Home Administrator
